How to Report an Injury

  1. To be scanned back to Email Workers Compensation

    • Please sign the Employee Acknowledgement of the Alliance Direct Contracting Program and scan it back.  This is to acknowledge that TASB has a medical network an injured employee has to stay within.  The Notice to read is included.

    • Please sign the Form to Elect Leave Benefits with Workers' Compensation.  This form lets us know if you wish to use leave days for any missed days you may have related to your injury.  You can change this form at any time.

    • Two page Injury Report: This is in the information packet.  This provides us with details of how the injury occurred.

  2. If you wish to seek medical care:

    • Please take the Notification of Employment for a Reported Workers' Compensation Injury of Illness form with you (in your packet) to one of the providers on the included list (in your packet) or from the website on the list. The website on the suggested list of providers is for you to check for other providers in the network that may be closer to your home.  The suggested list is local for this area.  You can fill out the top and you should not need an issuing signature.  It has contact and billing information for the provider or facility for TASB.

    • You need to tell the provider it is work related and they should give you a DWC73 form to bring back to your supervisor and if you can please scan a copy to Email Workers Compensation.  This form will let you know if you can return to work and, if so, what your restrictions are.

    • There is a prescription card in the packet if you are prescribed medication.

    • Please let us know if your supervisor cannot accommodate your restrictions and if you lose any time from work.  We need to track any scheduled missed time.  We can send you additional information on the lost time process when it occurs.

  3. If you have a loss of time due to your injury:

    • Notify your supervisor and your Absence Management representative if they usually put your leave in for you. Put the time in as your personal leave.  We can change the leave status if needed after we discuss the Leave Form.

    • Notify Email Workers Comp  so that we can provide you with a Leave Form to sign (if you have not already) so that we know if you will be using your leave or being docked for the first 7 days.  Workers Compensation will pay on your 8th day of missed work (does not have to be consecutive) at 70% of your earned pay. An additional email will be sent that will go into more detail as to how leave works related to Workers' Compensation.
