A Notification of Leave must be on file for all staff applying for Family Medical Leave.
You must log in to the Employee Portal and select Laserfiche Forms.

The required form is listed as Family and Medical Leave Request.
If you need to apply for Family Medical Leave, please complete the Medical Certification Form.

A medical release is required for all employees returning to work from leave. Medical Release Form

Prosper ISD offers a Catastrophic Sick Bank to all full-time employees that decide to enroll. The purpose of the sick leave bank is to provide additional sick leave days in the event of a catastrophic medical condition of the employee or the employee's immediate family. 

If you are a member of the Catastrophic Sick Leave Bank and need to apply for additional sick leave days, please complete the following forms.  Once both forms are received and reviewed, you will be contacted with further information.  Please read the Catastrophic Sick Leave Bank Guidelines before applying.
